The hospital needed help. As a cardiologist trained in critical care, I was called to the COVID ICU&#8212;embedded with pulmonary critical care colleagues to extend the reach of strained teams. </p><p>These were the early days of COVID&#8212;the days when N95s were in short supply, hallways were empty, ICUs were full, the full weight of exponential case growth bore upon us, and no one knew when the wave would crest and the fever break. We saved our N95s in biohazard bags, which in turn were stashed in brown paper bags, which in turn had our names scrawled across them in black Sharpie. There were barely enough masks to go around. </p><p>Because the workforce itself could not afford to get COVID&#8212;that would stretch thin resources even thinner&#8212;we were very, very careful about <em>who</em> went into infected patients&#8217; rooms. One member of the physician team would examine patients every day. Just one. Usually once.</p><p>But the nurses went in constantly. Every shift, multiple times. Turning patients. Adjusting drips. Checking the ventilator. It's elegant. It gives us the levers: improve the pressure difference, lower the resistance.</p><p>But what we didn&#8217;t examine was an even more fundamental&#8212;even existential&#8212;question: <em><strong>what is flow for?</strong></em></p><p>Why does the heart do its job at all? </p><p>Why does the cardiovascular system exist?</p><p>The very name, cardiovascular system, suggests an answer, a mode of operation. The heart is the leading organ in this setup. Cast as the conductor it orchestrates flow of blood throughout the body. The sovereign organ, crowned with coronaries.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-1" href="#footnote-1" target="_self">1</a> The ruler setting the Purpose</h3><p>The vascular system is not just the other limb of the cardiovascular system. It is its reason for being. </p><p>From the aorta&#8212;a surging Amazon of pressure and flow&#8212;all the way to the microscopic capillaries that trace their way through every organ, and back to the venae cavae which return blood back to the heart, the vasculature is the delivery network that enables the flow of blood.</p><p>And within this setup, the capillaries are the structures that suffuse organs and come into actual contact with the cells that do real work. For example, in the kidney, the capillaries come into contact with nephrons, organelles that filter blood, removing waste products and generating urine. In the liver, capillaries encounter hepatocytes&#8212;liver cells&#8212;which store energy and redistribute it, generate life-sustaining proteins, and also participate in toxin breakdown. </p><p>The same story in the skin that covers, the brain that thinks, and the limbs that move.</p><p>Throughout the body, the capillaries meet the cells that do the work of living.</p><p>The heart exists to make this possible. In fact, the heart exists <strong>only</strong> to make this possible. The heart has no other job.</p><p>Without the body, the cardiovascular system has no point. </p><p>And without the vasculature, the heart has no point. </p><p>The heart is not the purpose of the cardiovascular system. It is the servant of the purpose. </p><p>Silent and relentless, the heart&#8217;s job is not to beat, but to get energy to arms that hold a grandchild tight. </p><p>The ultimate servant organ.</p><div Frame</h3><p>We tend to think of capillaries as immaterial. Structures so small that they are quite literally microscopic. Considered in balance against the vibrant, restless, majestic, beating heart, capillaries just don&#8217;t seem to measure up. They don&#8217;t capture the imagination. On Valentine&#8217;s Day, we don&#8217;t draw capillaries&#8212;we draw hearts. Our imagination is captured by the heart, and the capillaries are relegated to a side story.</p><p>This hierarchy is backwards.</p><p>Nothing matters more than the capillaries in the cardiovascular system. Without them, nutrients don&#8217;t arrive. Waste doesn&#8217;t leave. The system has no exchange. The capillaries occupy the place of purpose in the cardiovascular system.</p><p>But what if we called it the vasculocardiac system instead?</p><p>We&#8217;d name the purpose first. The function first. The front line first. Instantly, the true hierarchy of action would come into sharp focus. We would understand that the vasculature is the business end of the circulation, the front line, the place of honor in the circulation.</p><p>We would understand that the point of the vasculature is to ramify into capillaries, networks which reach into organs where the work of the body is done. And we would in turn understand that the point of the heart is to generate the flow the vasculature needs to speed blood through capillaries so organs can get deliveries of nutrition, and drop off cargoes of waste.</p><p>We would understand that the point of the heart is to serve. And therefore we would not see the heart as a conductor, but as the conducted: led to serve. A power source responding to the needs of organs, not dictating them. </p><p>We would see the truth:</p><p>The cardiovascular system is structured to serve the front line.</p><div heart does not command. It listens. </p><p>It listens through baroreceptors, which detect shifts in pressure; through chemoreceptors, which monitor oxygen, carbon dioxide, and pH; through stretch receptors in the atria and ventricles; through autonomic tone set by the brainstem, which integrates moment-to-moment inputs from posture, respiration, digestion, emotional state, and rest. </p><p>The heart listens constantly, but not consciously. It does not issue instructions. It receives requests. And it honors these requests faithfully, responsively. It does not ask if the requests are valid. It just acts.</p><p>The heart senses demand through cues that are local, specific, and silent. Whispers heard in the tissues requiring more blood flow, or else passed from cell to nerve, and nerve to vessel. </p><p>These are not top-down orders. These are bottom-up truths. </p><p>When carbon dioxide rises, when oxygen levels drop, when muscles strain for fuel, or when the gut works to digest, the heart does not hesitate.</p><p>It leaps in response. It beats with more force or more frequency to facilitate faster flow. And then, just as naturally, it recedes. When the need passes, the heart slows and rests. </p><p>Not control. Contribution. </p><p>The heart does not demand compliance from the body. It meets the body&#8217;s needs with faithfulness.</p><p>It is not the sovereign of the system. Circulations to Organizations</h3><p>What does leadership that works this way look like? </p><p>It looks like executives, directors, and chiefs understanding their roles not as orchestrators, but as clearers of resistance, and optimizers of pressure. It looks like responsiveness, and enabling flow of resources, ideas, and energy throughout the organization. It looks like understanding that culture, identity, and purpose arises organically from within an organization: curated, perhaps, by leadership. But never controlled.</p><p>Servant leadership looks like understanding that leaders are not the originators of value, but enablers of those who deliver it.  </p><p>In the body, the capillaries are easy to miss. They don&#8217;t call attention to themselves. They let one red blood cell through at a time. In fact, sometimes we only notice them when they bleed or when they are found to be missing from an organ.</p><p>So too with the clinical front line unless we take the time to recognize and honor the essential work that they do. The front line is relentless and committed. They are where healing happens. The people who do.</p><p>Our job as leadership is not to take the spotlight. It is to deliver flow. To remove obstacles. To lift pressure. To serve.</p><p>We may still call it the cardiovascular system.</p><p>But we would do well to think of it as something else:</p><p>The vasculocardiac system.</p><p>Because the capillary is where purpose lives. The terrain is pretty flat. Driving down the highway, I mark the passage of towns by the parade of water towers&#8212;each one emblazoned proudly with a town name, sometimes a motto.</p><p>These towers rise quietly above the landscape, storing potential energy in the form of water held high. The higher the tower, the greater the pressure. And the more the energy. That energy is potential. Quiet. Still. </p><p>Until a faucet somewhere in town opens up, and water <em><strong>flows</strong></em>. </p><p>The more the faucet opens up, the lower the resistance, and the faster the flow. </p><p>No one really thinks much about a water tower. At least I don&#8217;t. It is elegant in its simplicity and design. Water is pumped up into the tower, but everything that happens after that is literally downhill&#8212;<em>until it isn&#8217;t</em>. </p><p>The water tower powers the circulation of water in a city. </p><p>The heart powers the circulation of blood in the body. </p><p>Those who work in healthcare &#8212; from the front desk to the back office, and all the clinicians in between &#8212; power the We must understand the <em>change</em> in pressure to understand flow.</p><p>In your house, that might mean the difference in pressure from the water main to the showerhead. In the circulatory system, it might be the difference between the high pressure in the left ventricle where the systemic circulation begins and the low pressure in the right atrium where the heart accepts blood to begin the cycle anew.  </p><p><strong>Resistance, R</strong></p><p>Resistance in the system, R, is anything in the system that impedes flow. Think rusty pipes, kinked hose, turning the faucet to a trickle. In the heart, resistance to flow might be a clogged artery or a narrowed heart valve. The higher the resistance, the more pressure is needed to generate the same flow.</p><p><strong>And finally, </strong><em><strong>Flow</strong></em><strong>, Q.</strong><a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-2" href="#footnote-2" target="_self">2</a></p><p>Flow is what we are here for, the star of the show. It is fundamentally defined as the quantity of something moving through a system per unit time. It&#8217;s not just how much there is, it&#8217;s how much is <em>moving and how quickly</em>. </p><p>In the cardiovascular system, flow is often expressed as liters of blood flow per minute (L/min). </p><p>In healthcare systems, flow might be expressed as patients per day, or procedures per year. While these volume metrics are critical for linking clinical care to business and operational metrics, in subsequent posts I would like to explore the distinction between <em>volume metrics</em> and <em>value metrics</em>. </p><p>A clinic appointment may register as complete, but if no therapeutic change is made&#8212;if no trust is built, no behavior changed, no preventive therapy prescribed, no meaningful decision advanced&#8212;has health really flowed? Has the healthcare system added value for the patient? Has the health of the patient flowed? And it&#8217;s simply a rearrangement of the equation above that really highlights what we care about, flow:</p><div class="latex-rendered" data-attrs="{&quot;persistentExpression&quot;:&quot;Q = \\frac{&#916;P}{R}&quot;,&quot;id&quot;:&quot;MBJSLKOQCJ&quot;}" data-component-name="LatexBlockToDOM"></div><p>Written this way, the levers that increase or decrease flow just pop.</p><p>A good equation should work like a machine&#8212;a machine that helps you simulate inputs and outputs in your mind.</p><p>This machine tells us clearly that if you want more flow through the circulation or any other system, there are two options. </p><p>One option is to increase the change in pressure across the circulation, for example by increasing how hard the heart squeezes to raise the systolic blood pressure, the peak pressure attained in the circulation after each time the heart squeezes or systole. This increases the driving pressure, allowing blood to make its way more forcefully through the circulation.</p><p>The other option is to decrease resistance in the circulation. For example, if the aortic valve is narrowed, preventing blood from ejecting out of the heart and into the circulation, a new valve can be placed to reduce resistance and improve flow. </p><p>I teach medical students that Q = &#916;P/R is the Central Dogma of Cardiology, and that virtually everything I do in clinical practice is geared toward improving blood flow using this equation. There are nuances and complexities, but cardiovascular medicine really does distill down to this. </p><p>What I&#8217;ve learned in over a decade of administrative leadership is that healthcare systems, at their core, operate on the same principle: <strong>a value-creating flow of health</strong>. </p><p>That flow can be unleashed&#8212;sometimes dramatically&#8212;by reducing resistance and improving the system&#8217;s capacity to generate and withstand the pressures of clinical demand. </p><p>Health systems like human bodies are complex, layered, nuanced&#8212;and unique. Function and dysfunction can seem mysterious in both. </p><p>What continues to astonish me is how often the patterns of flow and failure in the circulation have direct, elegant analogies in the way care moves&#8212;or stalls&#8212;through an organization. In this model, we imagine blood or patients moving through static channels, driven by pumps, regulated by pressure and resistance. </p><p>But this is only half the story.</p><p>The circulatory system is not made of static plumbing. It is lined with millions of <em>endothelial cells</em>&#8212;the front-line responders of the vascular world. Far from passive, these cells sense the contents and velocity of the blood flowing past. They release nitric oxide in response to shear stress, call down airstrikes of inflammation when they sense that something&#8217;s wrong, and regulate vascular tone, and in turn resistance, in real time. They are sensitive, dynamic, and deeply responsive to what flows through them. They are alive.</p><p>Healthcare systems are no different.</p><p>They are not made of software and schedules alone. They are made of <strong>people</strong>&#8212;frontline caregivers, coordinators, access specialists, medical assistants, nurses, techs, physicians. These human beings are not inert walls through which care moves. Like endothelium, they sense pressure. They adapt. They fatigue. They respond to what flows by&#8212;whether it&#8217;s the volume of patients, the emotional tone of interactions, or the complexity of the care required.</p><p>And just as endothelial cells are supported by the rest of the vessel wall, frontline caregivers in healthcare systems are supported by the rest of the healthcare workforce backing them up. Because for everyone else in healthcare&#8212;those who don&#8217;t touch patients directly&#8212;<em>the frontline is the patient.</em></p><p>And just as turbulent or toxic blood flow damages the endothelium over time&#8212;misaligned or unsustainable demands on frontline caregivers wear down the human side of the system. Burnout is not a moral failing. It is often the result of moral injury, precisely analogous to endothelial injury.</p><p>If we want to improve flow&#8212;of blood or of health&#8212;we can&#8217;t just look at <strong>what</strong> is moving through the system. We must also attend to <strong>how</strong> the system itself is changed by what it carries. </p><p>So health doesn&#8217;t just flow through a system. It <strong>shapes</strong> the system. The clinic is winding down. I end at 5. Will I stay on time? Will the patients arrive on time? I can feel the staff wondering.</p><p>It&#8217;s spring in Chicago, and fair weather on the weekend is not a given. It is a gift.</p><p>The electrocardiogram hangs on the door. Squiggles of electricity traced across the page give me a 10-second, 12-lead window into the heart I am about to examine&#8212;a window, in turn, into the person I am about to care for. </p><p>A 75-year-old woman with chest pain. That heart has beat over 2 billion times, and I will be listening to it for the first time. </p><p>What a privilege, I think to myself, in the less harried moments of the job. </p><div A fraction of a second&#8212;less than a heartbeat&#8212;but always a pause. </p><p>It&#8217;s a quick mental check. Am I ready? Are they ready? <em>Here we go.</em> </p><p>I snap into a ritual, a script seared now into my subconscious, practiced tens of thousands of times. Neurons synapsed so tightly together now that I doubt I could do it any other way. Pull the EKG off the door. Interpret it. Transfer it to my left hand, then knock twice briskly with two knuckles backhanded just before grabbing the handle of the door. <em>Twist, open, and hello.</em></p><p>For me, the clinic visit&#8212;pre-charting aside&#8212;has just begun. </p><p>For the patient, the journey has already been punctuated by landmarks like the call center, registration, scheduling, check-in, greeting, medication reconciliation, vitals, and a brief discussion of the chief concern. I am one part of an arc of experience that will continue through checkout, scheduling, and further testing.</p><p>In other words, depending on whether you are the clinician or the patient, even the definition of what the encounter comprises is different. </p><p><strong>Which brings us to our first insight:</strong> <em>When contemplating the true value of a clinic appointment, we must first ask: whose perspective are we evaluating &#8212; the patient&#8217;s, the clinician&#8217;s, the health system&#8217;s, or the payor&#8217;s?</em></p><p class="button-wrapper" data-attrs="{&quot;url&quot;:&quot;https://kannanmutharasan.substack.com/subscribe?&quot;,&quot;text&quot;:&quot;Subscribe now&quot;,&quot;action&quot;:null,&quot;class&quot;:null}" data-component-name="ButtonCreateButton"><a class="button primary" href="https://kannanmutharasan.substack.com/subscribe?"><span>Subscribe now</span></a></p><p></p><h3>A Quick Aside: Why Kick Off CardiacFlow with Clinic Appointments?</h3><p>Most of my practice time is now spent in the outpatient, ambulatory setting. This is a sustained trend in cardiovascular medicine. And so, since the clinic visit is the centerpiece of the ambulatory experience, it is the main &#8216;section of pipe&#8217; for the flow of a patient with cardiac conditions through the healthcare system.</p><p>To understand flow through the healthcare system&#8212;and we all want patients to flow through a seamless clinical experience from clinical concern to excellent clinical outcomes&#8212;we have to ask two questions:</p><ol><li><p><strong>What exactly is flowing through the healthcare system? </strong></p></li><li><p><strong>What are the parts of the system that produce value along the way?</strong></p></li></ol><p>What flows through the healthcare system is not volumes on a clinical volume dashboard.</p><p>What flows through the healthcare system is not cash on a cash flow statement.</p><p>What flows is not concentric circles of reach on a zip code map.</p><p>What flows is not even patients themselves.</p><blockquote><p>What flows is the patient&#8217;s health&#8212;across all dimensions: <strong>emotional, psychological, intellectual, social, and biological.</strong><em> </em></p></blockquote><p>Our mission is to elevate these dimensions at every step of the journey. </p><p>And that mission? Uphill.</p><p>Like generating ATP in the electron transport chain, powered by the finances and effort of patient and payor; sustained by the empathy, coordination, and clinical skill of the clinician The following table is my attempt at distilling the value of a clinic visit from the perspectives of patients, clinicians, the health system, and the payor. The clinical history of present illness (HPI), where the patient and clinician construct a narrative elucidating the chief concern, is the lynchpin of all clinical encounters. In training, I was taught that 75% of diagnoses can be made on the history alone. </p><p>In practice, this feels right. </p><p>When a patient relates their history&#8212;or when I&#8217;ve shared mine&#8212;the story really matters. Storytelling is how we relate to each other, how we remember one another, and how we honor each other. <em>Telling one&#8217;s story is how one feels seen. And telling stories takes time.</em></p><p>From the clinician's perspective, storytelling is very important to understanding more about the patient. At the same time, there is a tension: time is limited. The history is not just a way to relate to patients; <em>it is a clinical tool, just like a scalpel.</em> With the right training, it can be wielded with astonishing, surgical precision. </p><p>So sometimes, the questions must come staccato&#8212;when the diagnosis does not reveal itself in the course of the story. The clinician holds a cloud of possible diagnoses in their mind. </p><p>The differential diagnosis, we call it. </p><p>And just like the history is the lynchpin of clinical encounters, the differential diagnosis is the lynchpin of all medical reasoning. Everything comes down to arriving at the correct diagnosis. Because once you have the diagnosis, developing a treatment plan becomes much more streamlined. </p><p>And winnowing the differential diagnosis rests on deploying key questions known as pivotal questions&#8212;with the critical caveat that in the course of a patient telling their story, they may very well answer everything you want to know to establish a diagnosis, and may even share with you clues that you didn&#8217;t initially think to consider.</p><p>Let me illustrate this a bit&#8212;how storytelling and clinical reasoning meet. </p><p>Back to the 75-year-old woman with chest pain.</p><p>This is how I might think through her symptoms, for example:</p><ul><li><p>Is the pain worse when you take a breath in? &#8594; Could be pleuritic.</p></li><li><p>Worse with exertion? &#8594; Possibly cardiac.</p></li><li><p>Better when you then rest? &#8594; Even more likely cardiac.</p></li><li><p>Worse when lying down? &#8594; Pericarditis rises on the list.</p></li><li><p>Better when sitting forward? &#8594; Pericarditis moves even higher.</p></li></ul><p>This discussion highlights an important point: the history is a mechanism for generating bonding, understanding, knowledge, and trust for the patient. For the clinician, the history <em>can</em> be all of these things, but it absolutely <em>must</em> be a properly calibrated clinical tool.</p><p>And this is what everything converges upon: <em>trust, the invisible yet entirely palpable current that makes the entire system flow.</em></p><p class="button-wrapper" data-attrs="{&quot;url&quot;:&quot;https://kannanmutharasan.substack.com/subscribe?&quot;,&quot;text&quot;:&quot;Subscribe now&quot;,&quot;action&quot;:null,&quot;class&quot;:null}" data-component-name="ButtonCreateButton"><a class="button primary" href="https://kannanmutharasan.substack.com/subscribe?"><span>Subscribe now</span></a></p><h3>Trust.</h3><p>Trust is the silent current that suffuses every aspect of the clinic visit, just like blood flow suffuses every element of the body.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-1" href="#footnote-1" target="_self">1</a> And when it comes to contemplating value in the clinic visit, <em>trust is a force multiplier.</em> </p><p>Trust is what allows patients to tell the full truth about their symptoms, not just what they think the doctor wants to hear. Trust is what allows clinicians to ask the hard question, take the extra moment, or challenge an assumption&#8212;without fear of breaking the bond. </p><p>Trust is what allows health systems to work toward healing, not just throughput. </p><p>And trust is what allows payors to believe that the care being delivered is not just coded correctly, but clinically justified and necessary.</p><div It&#8217;s not a line on a dashboard. It doesn&#8217;t have a billing code. It doesn&#8217;t show up on the balance sheet. Yet its absence is immediately felt&#8212;in delays, confusion, friction, second-guessing, and non-adherence. The lack of trust clogs up every gear. <em>The lack of trust, like atherosclerosis on the inside of an artery, slows down flow. The lack of trust can even block flow outright.</em></p><p>On the other hand.</p><p>When trust is present, things move. Questions are answered more honestly. Histories are richer. Diagnoses become clearer. Plans are better followed. Outcomes improve. Experience is elevated. Velocity ensues. Systems flow.</p><p>That moment of pause before I knock on the door? </p><p>That&#8217;s not just a breath. It&#8217;s a reset, a silent promise: I am about to enter a sacred space where trust must be built quickly and authentically. I must be me, fully, authentically me. And I must endeavor to see my patient fully, too. Regardless of length or complexity, every clinic visit is a human encounter wrapped in a logistical, financial, and time-limited container. It can only work when trust flows freely through it.</p><h3>In Closing.</h3><p>I hope I have shared a few insights about value, trust, and flow within the relatable example of the clinic visit. Here we have had a glimpse of themes I would like to further develop as this conversation evolves:</p><ul><li><p>Value depends on who is asking the question. </p></li><li><p>The healthcare system is an active conduit to improve the broadly defined health status of patients. But none of this is possible without trust.</p></li><li><p>Trust&#8212;living, breathing, irreducible human trust&#8212;coursing through the healthcare system connects all stakeholders within it. is to help build health systems with soul: systems where patients get care like we would want for our own families, and where those who work in healthcare find deep satisfaction in the act of caring. Engineering), medicine (MD, Northwestern University Feinberg School of Medicine), discovery science (postdoctoral fellowships at Northwestern University Feinberg School of Medicine), and business (MBA, Northwestern University Kellogg School of Management). The people who know that healthcare is messy, dynamic, and human yet believe that structure, flow, and soul can be engineered into the system and grown in people.</p><p>I am a pragmatist at heart. This space is not about high-level theorizing decoupled from practical realities. Nor is it about oversimplified solutions that ignore the textured realities of healthcare delivery.
